Our case management services are designed to support individuals and families facing challenges through connection to necessary resources and services. Resource navigators work with you to identify your unique needs, then design action plans that build solutions to meet those needs. Services may include the following:
Case management programs serve individuals and families who have a low income and reside in the following zip codes: 92037, 92093, 92106-92111, 92117, 92119-92124, 92126, 92130-92131, 92140, 92145, 92161.

We may offer some limited assistance to individuals and families who meet certain eligibility criteria. Assistance is not provided on an emergency basis and the process typically takes at least one week. We are unable to contribute to past due rent.
